Health Care Directive Infomation Sessions – Melrose, CentraCare Health Big Birch Room

Melrose • CentraCare Health and Light the Legacy offer free advanced health care directive information sessions from 2 to 3:30 p.m. in Big Birch Room at CentraCare Health. Certified facilitators and a notary public will be present to provide forms, answer questions and assist in completing directives. (A health care directive is a planning tool used to identify the person or people one wants to make health care decisions for them if they aren’t able to communicate.
