Presentation (Advanced Health Care Directive) – St. Cloud, CentraCare Health Plaza, Windfeldt Room

St. Cloud • CentraCare Health and Light the Legacy offer free advanced health care directive information sessions from 5:30 to 7 p.m. in Windfeldt Room at CentraCare Health Plaza. Certified facilitators and a notary public will be present to provide forms, answer questions and assist in completing directives. (A health care directive is a planning tool used to identify the person or people one wants to make health care decisions for them if they aren’t able to communicate.
